Interleukin (IL-6)

Inflammatory markers include interleukin-6 (IL-6), serum C-reactive protein (CRP), serum calcitoninogen (PCT), heparin-binding protein (HBP), and serum amyloid A (SAA). Interleukin-6 (IL-6) belongs to the interleukin family and is a glycosylated protein. It is a polypeptide chain consisting of 184 amino acids with a molecular weight of 26 KD. IL-6 plays a central regulatory function in the process of infection and inflammation, and is one of the important inflammatory response factors. As a pleiotropic cytokine, IL-6 can be synthesized and secreted by various types of cells, including T cells, fibroblasts, monocytes, and endothelial cells involved in various cellular functions.

Structure of IL-6

 

 

Interleukin-6 (IL-6) is a small molecule glycoprotein with a molecular weight of 19-28 kDa, consisting of 184 amino acids in four alpha helices, usually in monomeric form, and with an isoelectric point of 5.0.The gene encoding IL-6 is present on human chromosome 7p15-21, which contains an intron (4) and an exon (5).There are three receptor-binding sites, including one specific receptor, and one specific receptor for IL-6. IL-6 has three receptor binding sites, including one specific receptor IL-6R (IL-6 binding receptor protein, IL-6R) binding site, and two gp130. (signal-transducing protein) binding sites.

IL-6 signaling mode

 

Interleukin-6 (IL-6) exerts its effects through three different signaling modes: classical signaling, trans-signaling, and trans-presentation. When classical signaling occurs, IL-6 binds the complex of mIL-6R and gp130, which then activates the JAK-STAT3 pathway for signaling. When trans signaling occurs, IL-6 binds to sIL-6R in vivo. And in trans-presentation, mIL-6R on dendritic cells binds to IL-6 and is presented to T cells expressing gp130 on the surface.This alternation of IL-6 signaling is an important event in the initiation of helper T cell 17 (Th17).

 

Biological role of IL-6 in inflammation and immunity

 

The concentration of interleukin-6 (IL-6) in the circulation of a normal organism is extremely low, about 1-5 pg/ml. when an inflammatory response occurs, the concentration of IL-6 rises dramatically, and in the case of sepsis, it can reach the ug/ml concentration range. In the early stages of inflammation, after IL-6 is produced at the local lesion site, it first enters the liver through the blood circulation, followed by the rapid induction of the production of acute temporal proteins, such as CRP, FGG, SAA, and HP.On the other hand, IL-6 decreases the synthesis of fibronectin, albumin, and transferrin. When the body is exposed to high levels of SAA for a long period of time, the formation of amyloid A causes serious complications in chronic inflammatory diseases.
