Cardiac Troponin I (CTnI)

Troponin I (TnI) is one of the three subunits that make up the thin filament troponin complex of rhabdomyosin. Cardiac troponin I (CTnI) is a cardiac-specifically expressed regulatory protein that plays a key role in the contraction of the heart by inhibiting the binding of myosin and Actin, which mediates the relaxation of the rhabdomyosin.TnI-cardiac is one of the three genes of the TnI subfamily. This gene encodes the TnI-cardiac muscle protein, which is expressed only in myocardial tissue. Mutations in its gene can cause familial hypertrophic cardiomyopathy type 7 (CMH7) and familial restrictive cardiomyopathy (RCM).

Numerous studies have shown that cTn levels change in the early stages of acute myocardial infarction (AMI) by a wide range. cTnI is one of the most sensitive and specific serum markers of cardiomyocyte injury.

 

Molecular structure and characteristics of CTnI

 

Cardiac troponin (cTnI) consists of 210 amino acids, with a molecular weight of 23.9 and an isoelectric point of 9.87. cTnI, as a protein with an a-helix, has five helical regions in a stretched conformation. cTnI is found in human cardiomyocytes in two forms, cTnI-C-T complex, and free cTnI. cTnI is found as a small portion of soluble protein in the periplasm. free protein; otherwise, most of the cTnI is an insoluble structural protein that is integrated into myofibrils. However, no form of cTnI can pass through the intact myocardial cell membrane and enter the blood circulation, and therefore the amount of cTnI in the blood is extremely low. When the integrity of the cell membrane is damaged by hypoxia and ischemia, free cTnI can quickly penetrate the damaged cell membrane and enter the blood circulation; at the same time, cTnI fixed in myofibers is slowly broken down and becomes free cTnI. cTnI, as a diagnostic marker for acute myocardial infarction, can not only be detected at an early stage but also lasts for a longer period.
