CD278 (Gene ID: 29851) was identified as a member of the CD28/CTLA-4 co-stimulatory molecule family in the early 21st century. Its expression is inducible upon activation of T cells, thus earning it the name "inducible T-cell co-stimulator". CD278 interacts with its ligand, ICOS ligand (ICOS 1 or B7h), and is expressed predominantly on antigen-presenting cells (APCs) such as dendritic cells and B cells. This interaction provides co-stimulatory signals that enhance T cell activation and function.The main functions of CD278 are as follows:

1. T cell activation: CD278 promotes T cell activation by participating in enhanced T cell receptor (TCR) signaling. It leads to increase production of cytokines, including IL-4, IL-10 and IL-21, which are essential for various immune responses.

2. T follicular helper (Tfh) cell differentiation: CD278 is essential for the differentiation and function of Tfh cells, which are critical for providing B cell assistance during the germinal center reaction. The massive expression of CD278 by Tfh cells is essential for the production of high-affinity antibodies.

3. Antibody response: CD278-mediated Tfh cells are essential for facilitating antibody class switching and promoting affinity maturation in B cells. Deficiencies in CD278 or ICOSL lead to impaired antibody response, thereby increasing susceptibility to infection.

4. Regulatory T cell (Treg) stability: CD278 is expressed on Tregs and plays a role in maintaining their stability and suppressive function. Disruption of CD278 signaling can lead to dysregulation of the immune response and autoimmune disease.

5. Immune tolerance: CD278 is associated with immune tolerance and autoimmune prevention. Dysregulation of CD278 signaling may lead to breakdown of immune tolerance mechanisms.

6. Immune-related diseases: The aberrant signaling of CD278 has been associated with autoimmune diseases such as systemic lupus erythematosus (SLE) and rheumatoid arthritis, highlighting its importance in maintaining immune homeostasis.
